Every wood fire leaves creosote behind. It builds on the flue walls in layers, and once it hardens into a glazed crust it becomes fuel sitting directly inside your chimney. That is the single most common cause of a chimney fire.

Freeze and thaw cycles do real damage to Central Virginia brickwork. Water gets into a hairline crack, freezes, expands, and the crack widens. A few winters of that and you have spalling brick, a failed crown or a leak showing up on the ceiling next to the fireplace.

White staining on the exterior brick, crumbling mortar joints, brick faces flaking off, a damp patch on the ceiling near the fireplace, a rusted damper, or a strong smoky smell when the fireplace is not in use. Any of those are worth a look.
Yes, and this one gets overlooked constantly. A furnace or water heater venting through a chimney is moving carbon monoxide through that flue. A blockage or a deteriorated liner in that situation is dangerous.
